He's a boy. He probably has schizophrenia.

OMG!

Salad fingers is a boy. ("A man can't cope with all these unpleasant frequencies about ....") Not once does he say he is a girl. He is just insane and confused. He must have been in his bunker when his town got bombed during the "Great War," and the radiation screwed him up. I think he is the only one left.

I believe that all of the "creatures" he encountered were figments of his imagination, and they were based on what feelings he was having at the moment. He had the slightest recollection of his old family but they are probably all dead. It's really sad, how the series ends with him crying in his bunker all alone in the middle of nowhere.
